a. Disability Services as provided by the The Office of ADA Services for Students at Millsaps College (PDF) - For your information and personal records.
b. Consent Form for FERPA, The Family Educational Rights and Privacy Act of 1974 (PDF) - Please read, complete consent form and return to the Office of Records.
c. Health Forms: Medical Report and Vaccination Information (PDF) - Please note: The Health History Form, TB skin test report (performed within the last year), and the immunization record are required for all entering students to complete registration. Please fill out the medical history completely.
3. Final Transcript - If you choose to enroll at Millsaps, please arrange to have your college counseling or guidance office send us your final high school transcript upon graduation.
